I intend to be legally responsible for my actions that may result in injury or damage to me, and do hereby release and discharge, on behalf of myself, my heirs, executors, and administrators, the Ministry of Tourism, Culture & Sport, the Sport Alliance of Ontario, ParaSport Ontario and the Ontario Blind Sports Association, their respective directors, officers, members, employees, coaches, volunteers, officials, participants, owners/operators of facilities, and representatives (i.e. the “Organization”) are not responsible for any injury, damage or loss of any kind suffered by a participant during, or as a result of any program, activity or event caused in any manner whatsoever including but not limited to the negligence of the Organization (Ontario Blind Sports Association) and its representatives. I consent to any medical treatment that may be administered as a result of my participation. I am aware that full disclosure of any prior medical conditions will be required in the event that treatment is administered for an injury at this event. I give permission for the free use of my picture, video and/or name for possible publicity, promotional or media purposes and constitute a waiver of any and all claims for compensation from all sponsoring agencies.
